Case management represents our commitment to offering quality,
community-based services to people and families infected and affected
by HIV & AIDS.
Case managers serve as advocates. They assist clients to navigate the
complicated public benefit systems and obtain access to needed
services, including HIV primary health care, OB/GYN services,
substance use treatment, child care, and permanency planning.
Referrals are made for Medicaid, SSI/SSD, housing, support groups,
food programs, legal services, ADAP, and other services for which they
are eligible
Case management services for the people with HIV/AIDS are offered at
our 56 Bay Street and 166 Port Richmond Avenue locations and in the
community.
We provide specialized case management services to families,
individuals, undocumented immigrants, people with mental illness and
men at the Arthur Kill Correctional Facility.
The following is a listing of our HIV Case Management & Supportive
Service programs-
COBRA Case Management
– This is an intensive case management program that assists
individuals and families living with HIV/AIDS to navigate the complex
medical, public benefits, and social service systems of NYC. A team of
case managers assist with advocacy; access to and ensuring the equity
of medical care and social services; family supportive services;
crisis intervention; individualized case work; and specialized
information & referral. Individuals served by this program are
Medicaid eligible.
Family & Pediatrics
Case Management
– This case management program assists expectant mothers, children,
and young adults (under age 24) living with HIV/AIDS. Service includes
advocacy; access to and ensuring the equity of medical care and social
services; family advocacy; crisis intervention; individualized case
work; and specialized information & referral. Services are delivered
by case manager and social worker (LMSW) in coordination with
specialized medical care.
CSP Case Management
– This case management program assists individuals and families living
with HIV/AIDS who have private insurance, are under insured, or have
no insurance. As a grant-funded program your insurance is not impacted
by utilizing this service. A case manager assists with advocacy;
access to medical care and social services; family supportive
services; crisis intervention; individualized case work; and
specialized information & referral. Brief, supportive case
management is offered for individuals who do not need ongoing case
management.
Undocumented
Immigrant Case Management
– This case management program assists undocumented immigrant
individuals and families living with HIV/AIDS on Staten Island in need
of support. The case management team assists with advocacy; access to
medical care and social services; family supportive services;
referrals for legal services; crisis intervention; individualized case
work; and specialized information & referral.
Arthur Kill
Correctional Facility Pre-Release Services
– This case management program assists HIV+ individuals incarcerated
in Arthur Kill Correctional Facility (AKCF) who are scheduled for
release. The case management services include discharge planning;
scheduling & escort to Parole; advocacy; scheduling & escort to
medical care and social services; crisis intervention; individualized
case work; and specialized information & referral. Advocacy and
support groups are offered to HIV+ individuals currently incarcerated
at AKCF.
HIV Criminal Justice
Initiative, Community Re-Entry Program – This program aims to reduce recidivism for HIV+ men just released from
a Department of Corrections facility. Services include transportation
from point of entry to NYC, escort & advocacy with Parole, medical
care, HASA, housing, CBO’s, and the completion of discharge plan.
Escorts &
Translations
– The service provides Spanish/English translation and escorts to
ensure HIV+ individual has access to medical care, social services,
public benefits, and other services needed to ensure the continued
health of the individual.
Treatment Adherence
Consultation
– This service assists individuals in making informed decisions about
their HIV medication and HIV medication and treatment, and provides the best available
information on therapies and adherence strategies.
